To expedite your initial visit with us, we encourage patients to download and complete our Patient Intake Forms and bring them with you to your first visit.

If you are unable to complete the forms prior to your initial evaluation, we ask that you arrive 15 minutes prior to your appointment time to complete the forms.

Your First Visit
It is common to feel apprehensive if you’ve never had physical therapy treatment. We recommend that you take the time to write down any question, concerns, and symptoms that you want to discuss with your physical therapist prior to your visit.
Please wear loose fitting clothing and sneakers to all physical therapy appointments, including your initial evaluation. This will allow your therapist to properly assess your posture and movement patterns.
Please bring the following items with you to your evaluation:
- Physician’s prescription for physical therapy
- Insurance card(s)
- Driver’s license or another form of photo ID
- Completed intake paperwork
The initial evaluation appointment will last approximately 1 hour. Your physical therapist will complete a thorough history of your condition, perform specific tests and measures, and discuss their findings and treatment plan with you. The therapist will then initiate your treatment and review your home exercise program. Before you depart, the front desk team will schedule future appointments based on the recommended frequency and duration of your treatment plan. We recommended scheduling 2 to 3 weeks in advance to ensure you get the time of day that works best for you.

Our Billing Team will verify your physical therapy benefits with your insurance and inform you of your coverage. If you do not see an insurance company on the above list, our team is happy to verify your coverage and determine if you can treat at Momentum Physical Therapy.
